No one else is reporting this problem.
The locations are in a "Tree" format that can be expanded and contracted. I am 99% certain that you simply need to click the Small triangle (Mac version) in front of "World" and then keep clicking triangles in front of "North America" and "Lower 48" to see the states. Click in front of "Canada" to see provinces.
For Windows, small + signs appear - just click on these to expand.
Similar expandable "trees" are used throughout the program -- such as the Groups tab in the ID Wizard.
This is an excellent question. We do not want folks getting "stuck or feeling frustrated when using the program. That is why we try to answer this sort of thing in the Help files or the Knowledgebase. However, the answer to this question was not in the Knowledgebase -- I am adding it today.
I am very interested in hearing of any other situations like this where anyone was frustrated. Even if you finally figured out something on your own, please let me know where we could do a better job of making the program easier to use. email Pete@thayerbirding.com with any comments.
